Action needed: Please update your child’s Health Consent Form in the Mission CISD FOCUS Parent Portal.
WHY IT MATTERS
The 89th Texas Legislature passed Senate Bill 12, requiring school districts to notify parents about health-related services and to honor a parent’s right to provide or withhold consent for those services.
WHAT PARENTS CAN DO
- Log in to the FOCUS Parent Portal.
- Open your student’s profile and select the Health Consent Form.
- The Forms will be located under the FORMS folder located under the left hand side of the screen. Look for Forms Summary.
- Review and submit your consent preferences.

- Click Here to Reset Password
- Then click “I have forgotten my password” on the FOCUS login page to reset.
Can’t access the portal? Call your child’s campus—
the attendance clerk can help you complete consent in person or by phone.
