What is transition?
Transition is planning for a student’s life after graduation.
Transition addresses the life domains of:
- Post secondary education and learning options
- Independent living skills
- Recreation/ leisure activities
Transition involves the student through interviews, inventories and having the student attend ARDs once he/she is 14. A student’s IEP must address transition by age 14.
When does transition planning begin?
Transition planning must begin on or before a student’s 14th birthday as mandated by state and federal policy.
MCISD provides transition inventories, check lists and Visual Transition Plans to identify student’s strengths, preferences, interests and needs. Ask your child’s teacher about these planning tools.
Why is transition important?
Once a student exits the school system, all services are based on eligibility instead of entitlement (as are school services). All services must be put in place by family, student and school staff, to avoid a student having no work or opportunities to learn, work or community
Families and schools should be sure to have the supports a student will need after graduation in place before exiting the school system to provide for a seamless flow of services.
What to consider in transition planning?
When will the student graduate? Students receiving special education services may stay in school until they are 21 if so desired.
Under what plan will the student graduate?
- Distinguished (26 credits)
- Graduate with Endorsements (26 credits)
- Foundation Program (22 credits)
- IEP Completion - age out
Will the student attend college, a trade school, or community classes such as ACC STEPS Program, UT Informal classes, ARCIL, fitness classes, etc.?
Will the student work and what resources will be needed to support successful employment?
What will the student do for recreation and social interaction?
What financial and personal supports will be needed?
What transportation services will the student need?
How will a student pay for medical services and how can the student move smoothly from pediatric to adult services?
What happens when the school bus stops coming?
Students with disabilities and their families often wonder what life will be like after graduation, and what supports will be available when school services end.
Transition planning is a process that’s designed to answer these questions. There are federal and state requirements for schools and ARD committees to address transition for students with disabilities, including:
- Transition assessments
- Postsecondary goals
- Annual IEP goals related to the student’s transition service needs
- Transition services to enable the student to meet their postsecondary goals
- Course of study aligned to student goals and interests (graduation plan)
- Appropriate student and parent involvement in the ARD meeting
- Transfer of rights at age of majority
- Connection to agencies and other resources that can support the student in adulthood
In Texas, a student’s IEP must address transition by their 14th birthday (TEC §29.011). This website provides resources for schools to understand and implement effective transition planning practices.
